Position Objectives:

As a Diabetes Care Dietitian Specialist based in Boca Raton, FL, you will be essential in supporting patients with diabetes through expert nutrition counseling and education. In collaboration with our healthcare team, you will develop and implement personalized care plans using a patient-centered approach to inspire meaningful lifestyle changes. This role is based in our corporate office, with occasional travel to our endocrinology clinics in South Florida. Bilingual candidates fluent in Spanish and English are preferred, but this is not a requirement.

Schedule:

 

  • Monday through Friday: 8:00 AM - 5:00 PM EST (in our corporate office)


Position Responsibilities:

  • Collaborative Care: Work with the healthcare team to integrate medical nutrition therapy into personalized diabetes management plans.

  • Nutritional Documentation: Accurately document patient interactions and nutritional care in medical records.

  • Care Plans: Develop and execute individualized nutritional care plans for diabetes management.

  • Patient Support: Use a patient-centered approach to guide and support positive lifestyle changes and adherence to care plans.

  • Evidence-Based Care: Deliver high-quality, evidence-based nutrition counseling tailored to each patient’s needs.

  • Progress Monitoring: Track and document patient progress toward diabetes management goals.

  • Continuous Learning: Stay informed about the latest advancements in diabetes care, technology, and research.

  • Educational Resources: Utilize a variety of resources to enhance patient education and engagement.

  • Education Delivery: Provide educational sessions for patients both individually and in group settings.

  • Remote Monitoring: Participate in StrideMD’s remote monitoring services with flexible hours when needed.


Qualifications:

  • Certification: Certified Diabetes Care and Education Specialist (CDCES) preferred but not required.

  • Experience: Minimum of 2 years in diabetes education and nutrition counseling.

  • Skills: Strong communication, organizational, and interpersonal skills. Familiarity with diabetes technologies and educational resources.

  • Cultural Competence: Ability to work effectively with diverse patient populations.

  • Language Skills: Bilingual in Spanish and English is preferred but not required.

  • Travel: Occasional travel to South Florida clinics as needed.
