Job Summary: 
We are seeking a compassionate and proactive Retention Agent to join our team, specializing in retaining patients enrolled in our Remote Patient Monitoring (RPM) program for diabetes management. The Retention Agent will focus on engaging patients, ensuring adherence to their care plans, and providing educational support to improve health outcomes. This role is essential for fostering patient satisfaction, encouraging active participation in the program, and maintaining long-term patient relationships.

Key Responsibilities:

Patient Engagement:
  • Proactively contact patients in the RPM diabetes program to check on their status and adherence to monitoring plans.
  • Provide motivation and encouragement to enhance patient engagement with health coaches and care plans.
  • Build and maintain strong relationships with patients to foster trust and continued participation.

Educational Support:
  • Provide educational resources to help patients understand and manage their diabetes more effectively.
  • Conduct one-on-one or group sessions to reinforce the importance of adhering to the monitoring protocols within the care plan.
  • Act as a key resource for patients, offering guidance on diabetes management and support.

Feedback Collection and Analysis:
  • Gather and analyze patient feedback to understand their needs, concerns, and satisfaction levels.
  • Use patient feedback to enhance engagement strategies and improve the effectiveness of the RPM program.

Collaboration:
  • Work closely with healthcare teams, including diabetes health coaches and clinical staff, to ensure patient care plans are communicated effectively and any issues are promptly addressed.
  • Collaborate with other departments to ensure a seamless and supportive experience for RPM patients.

Reporting:
  • Maintain accurate and detailed records of all patient interactions and outcomes in patient management systems.
  • Provide regular reports on patient retention metrics, challenges, and successes to relevant stakeholders.

Qualifications:

Education:
Degree in healthcare management, nursing, social work, or a related field. Additional training or certifications in diabetes management is a plus.

Experience:
Minimum of 2-3 years in patient retention, healthcare customer service, or case management, with a focus on chronic disease management preferred.

Skills:
  • Strong communication and interpersonal skills.
  • Proficiency with patient management systems and CRM tools.
  • Excellent problem-solving abilities with a creative approach to patient engagement.
